Full payment is due at time of service. Cash and checks are the accepted forms of payment. Credit cards are not accepted.
Insurance is not accepted. However, a statement which you can file with your health insurance carrier (if you choose to do so) will be provided.
The office maintains a 24-hour cancellation policy. Appointments cancelled with less than a 24-hour notice will be charged the full fee for the time reserved.
